Job summary

Horizon Home Care & Hospice is seeking a dedicated Home Care Nurse in Milwaukee, WI, to provide exceptional patient care. You'll assess patients' needs, develop care plans, and collaborate with a multidisciplinary team to enhance patient health outcomes.

This full-time role offers a structured schedule with no weekends or on-call duty, accompanied by a competitive pay structure including a hiring bonus of up to $1,000. Ideal candidates will hold a current WI RN license and have previous Med/Surg or Home Care experience.

Qualifications

• Degree from an Accredited School of Nursing (BSN preferred).
• Current WI RN license necessary.
• 1-2 years of Med/Surg or Home Care experience required.
• Current/Valid WI Driver's License; plus reliable/insured vehicle.

Responsibilities

• Complete patient plan of care and submit admission paperwork.
• Perform ongoing assessments and follow physician orders.
• Identify patient goals and develop implementation plans.
• Audit clinical records upon recertification and discharge.
• Coordinate ongoing services with Prior Authorization Nurse.
