Elliot Hospital is seeking an RN Case Manager to provide clinically-based case management and discharge planning support. You will collaborate with the care team to ensure safe transitions and resource utilization aligned with professional standards.

The role requires a BSN or progress toward one, 3–5 years of case management experience, and CM/ACM certification preferred. On-site with comprehensive benefits and opportunities for training and growth.

Qualifikationen

• Bachelor's degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieving the degree in a specified time.
• 3–5 years of case management-related healthcare experience.
• CM/ACM certification preferred.
• Excellent clinical documentation and EMR proficiency preferred.

• Assesses and identifies patients’ needs during discharge planning.
• Provide resources and community referrals for continuity of care.
• Care coordination with families, providers, and clinical team.
• Follows Medicare conditions when needed.
• Utilization Reviews.
