Job summary

The Elliot Hospital, part of Elliot Health System, is seeking an RN Case Manager to coordinate patient care and discharge planning. You will work with the care team to identify resource needs and ensure safe, effective transitions to community providers.

Responsibilities include assessing discharge needs, arranging referrals, and ensuring Medicare guidelines are followed, while collaborating with families and clinicians for optimal outcomes.

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieving the degree in a specified amount of time.
• 3–5 years of case management-related healthcare experience.
• CCM or ACM preferred.
• Excellent clinical documentation and EMR usage preferred.

Responsibilities

• Assesses and identifies patients’ needs during the discharge planning process.
• Provide resources and community referrals for continuity of care.
• Care coordination with families, providers, and clinical team.
• Follows Medicare conditions when needed.
• Utilization Reviews.
