Elliot Health System's Elliot Hospital is seeking an experienced RN Case Manager to coordinate patient care and ensure safe transition plans. You will work with the care team to identify resource needs and manage discharge planning.

The role emphasizes collaboration with families and providers, adherence to case management standards, and nursing ethics. Weekend availability is required; on‑site work at the Manchester, NH campus.

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieving the degree in a specified amount of time.
• 3–5 years of case management-related health care experience.
• CCM or ACM preferred.
• Excellent clinical documentation, and successful usage of EMR systems preferred.

Responsabilités

• Assesses and identifies patients’ needs during the discharge planning process.
• Provide resources and community referrals for continuity of care.
• Care coordination with families, providers, and clinical team.
• Follows Medicare conditions when needed.
• Utilization Reviews.
