Elliot Hospital in Manchester, NH seeks an RN Case Manager to provide clinically-based case management, ensuring efficient patient care and safe transitions. You will collaborate with the health care team to optimize resource use and address social determinants of health for each patient.

The role requires a BSN or progress toward the degree, 3–5 years of case management experience, and CCM/ACM certification preferred.

Competenze

• Bachelor's degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieving the degree in a specified amount of time.
• CCM or ACM certification preferred.

Mansioni
