Job summary

Elliot Hospital is seeking a RN Case Manager to provide clinically-based case management under the Director of Case Management. The role supports efficient patient care and safe transitions, collaborating with the care team to align resources with patient needs and social determinants of health.

The Case Manager will document thoroughly, follow Medicare guidelines when required, and participate in an inter-professional practice model.

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieving the degree in a specified amount of time.
• 3–5 years of case management-related health care experience.
• CCM or ACM preferred.
• Excellent clinical documentation and EMR usage preferred.

Responsibilities

• Assesses and identifies patients' needs during the discharge planning process.
• Provide resources and community referrals for continuity of care.
• Care coordination with families, providers, and clinical team.
• Follows Medicare conditions when needed.
• Utilization Reviews.
