Job summary

Qualifications

• Associate or bachelor's degree in nursing from an accredited institution.
• Active RN license in IL.
• 3–5 years of clinical nursing experience; 2–3 years in case management or related fields.
• CCM certification preferred.

Responsibilities

• Serve as liaison among members, families, providers, employers, health plans, and community organizations.
• Coordinate care for members with complex health conditions.
• Develop and monitor individualized care plans to improve health outcomes.
• Educate members on disease management and available resources.
• Monitor member progress and adjust care plans as needed.
