CVS Health in the Chicago area is seeking a Case Manager - Registered Nurse to serve dual eligible populations, addressing complex health and social needs with compassionate care and collaboration.

In this full-time role, you will coordinate benefits, develop health strategies, liaise with members, providers, employers, and insurers, document case work, and monitor progress while ensuring regulatory compliance. Travel up to 50% within Chicago ZIP codes 60616 and 60608.

Qualifications

• Associates Degree or Nursing Diploma (REQUIRED)
• Bachelor's Degree (PREFERRED)
• 3+ years clinical practical experience required
• 2+ years case management, discharge planning, and/or home health care coordination experience preferred
• Active and unrestricted Registered Nurse (RN) License in Illinois (IL) required
• Valid and active driver’s license required

Responsibilities

• Facilitate the delivery of appropriate benefits and healthcare information.
• Develop, implement, and support health strategies and policies.
• Act as a liaison with members, employers, providers, and insurance companies.
• Implement and coordinate case management activities for catastrophic cases.
• Interact with members telephonically or in person.
• Assess and analyze members' medical and vocational status.
• Communicate with stakeholders regarding care and treatment.
• Prepare documentation of case work activities.
• Monitor member progress toward desired outcomes.
