Qualifications

• Minimum 3-5 years clinical practical experience.
• Independent/remote work capable, with virtual collaboration.
• Excellent analytical and problem-solving skills.
• Effective communication, organizational and interpersonal skills.
• Ability to work independently.
• Proficient with MS Word, Excel, Outlook and PowerPoint, plus some proprietary apps.
• Preferred: 2-3 years Care Management, discharge planning or home health care coordination experience.
• Certified Case Manager
• Bilingual

Responsibilities
