Qualifications

• 3–5 years of clinical experience.
• 2–3 years in care management or discharge planning.
• Comfort working from home and using virtual collaboration tools.
• Strong analytical and problem-solving skills.
• Excellent communication and organizational skills.
• Ability to work independently.
• Proficiency with MS Word, Excel, Outlook, and PowerPoint.
• Familiarity with standard corporate software applications.

Responsibilities

• Develops a proactive plan of care to address identified issues and improve outcomes.
• Reviews data from claims and other sources to assess needs and benefits.
• Applies clinical judgment to reduce risk factors and barriers.
• Conducts assessments considering co-morbidity and multiple diagnoses.

• Collaborates with supervisor and care team to meet goals.
