Job summary

Abode Services is seeking a Nurse Case Manager to support Alameda County Programs. The role involves coordinating care with psychiatrists, administering medications, and completing assessments and treatment plans across home, community, and office settings.

The RN will drive to appointments, manage medications, and work with a multidisciplinary team to promote recovery and adherence to care plans. Strong documentation and flexibility are essential.

Qualifications

• Current California RN license is required.
• Drive to appointments and home visits; reliable transportation.
• Flexible hours including some weekends/evenings may be required.

Responsibilities

• Participate in treatment planning and implement services to support medication adherence.
• Administer medications and injections with coordination from psychiatrists and pharmacies.
• Assess nutritional needs and help improve general health.
• Provide clinical case management in home, community, and office settings using ACT model.
• Maintain the medication room and ensure compliance with certifications.
• Develop and update assessments and treatment plans with participants Foster recovery goals.
• Perform mental status and safety assessments (suicide, homicide, gravely disabled).
• Facilitate psycho-educational and support groups.
• Attend clinical, administrative and case conference meetings.
• Enter data timely to support program evaluation and outcomes tracking.
• Document services clearly using written case notes and Medi-Cal billing.
• Collaborate on participant care with Psychiatric NP/PA and staff.
• Assist Program Manager and clinical supervisors as needed.
• Other duties as assigned.
