Job summary

Abode Services is seeking a Nurse Case Manager to support Alameda County Programs. The RN will work with psychiatrists to help participants attend appointments, manage medications, and navigate pharmacy/insurance issues. The role includes home visits, assessments, treatment planning, and ongoing case management in community settings.

Qualifications

• RN license (California) required.
• Associate's or Bachelor’s Degree in Nursing.
• Valid CA driver's license and auto insurance.

Responsibilities

• Support participants with medication adherence and appointment attendance.
• Administer medications and injections in collaboration with clinicians.
• Conduct assessments, develop treatment plans, and perform home visits.
• Provide rehabilitative and case management services in home, community and office settings.
• Maintain documentation and data for program evaluation and billing.
