Job summary

Abode Services in Napa County seeks a Clinical Case Manager to support our Napa County Programs, providing clinical case management to increase wellness, housing stability, and reduce involvement with hospitals or the criminal justice system.

You will assess needs, develop service plans, perform mental status and safety assessments, deliver crisis counseling, and coordinate care with treatment teams while documenting in BIRP and supporting families.

Qualifications

• Clinical case management services in home, community, and office.
• Develop and update individual service plan goals with the treatment team.
• Perform mental status and safety assessments (including suicide, homicide, gravely disabled).
• Provide direct crisis counseling and needs identification.
• Utilize motivational interviewing to support abstinence from substances.
• Coordinate wrap-around clinical services for residents in permanent supportive housing.

Responsibilities

• Enter data timely to support program evaluation and outcomes tracking.
• Ensure documentation of services using the BIRP format.
• Attend clinical, administrative, and case conferencing meetings as assigned.
