Job summary

Health Care for the Homeless in Baltimore is seeking an RN Case Manager to provide medically-related case management within the Housing Services program. You will coordinate care, perform nursing assessments, conduct home and community visits, and liaise with medical providers to improve housing placement outcomes.

As part of a multidisciplinary team, you will deliver health education, support adherence, document in the electronic health record, and help develop integrated treatment plans using

Qualifications

• Bachelor’s Degree from an approved School of Nursing.
• Licensed in Maryland as a Registered Nurse
• Personal vehicle and valid Maryland driver’s license
• Two years of case management experience preferred
• Two years of clinical nursing experience
• Experience working with individuals who have behavioral health disorders preferred

Responsibilities

• Provide nurse case management services to clients in Housing Services, including medical care coordination and linkage to health resources.
• Deliver health education and counseling, exploring lifestyle choices and safety.
• Perform nursing assessments, treatments, medication administration and adherence support.
• Conduct home and community visits and escorts as per the care plan.
• Demonstrate client-centered, trauma-informed care and collaborative team engagement.
• Develop and implement integrated treatment plans using SMART goals.
• Complete documentation in the client’s electronic health record within required timeframes.
