Job summary

Yale NewHaven Health is seeking a Case Manager to coordinate and optimize patient care across the continuum, working with the interdisciplinary team to meet clinical and financial goals. You will ensure a smooth transition from inpatient to outpatient care while aligning with institutional objectives.

The role requires RN licensure in Connecticut, a BSN or higher, and at least three years of relevant clinical experience.

Qualifications

• Minimum of a Baccalaureate degree in clinically related field. RN required.
• Minimum of three (3) years of relevant clinical experience.
• Active RN Licensure in Connecticut.
• Knowledge of computer software and hardware applications and a basic knowledge of statistics.
• Advanced communication and interpersonal skills with all levels of internal and external customers.
• Advanced assessment and teaching skills.
• Leadership skills in planning and managing patient care in acute care.
• Utilization management and case management experience preferred.

Responsibilities

• Coordinate and ensure the implementation of the plan of care.
• Establish a system for coordinating care across the continuum.
• Optimize hospital system efficiency impacting quality or length of stay.
• Identify and monitor variances from clinical pathways or treatment plans.
• Use information from resources to meet patient admission and discharge needs.
• Assist clinicians in documenting admissions and continued stays.
• Manage Medicare notices of non-coverage and appeals.
• Develop discharge plans with care teams and ensure follow through.
• Refer to outside agencies and initiate needed referrals.
• Advocate for patients and families; connect to community resources.
