Job summary

The Jewish Board in New York is seeking an RN Case Manager to act as the clinical liaison between medical services, residents, and staff, ensuring nursing care aligns with OPWDD rules and JBFCS policies. You will supervise LPNs, AMAPs, and direct support professionals, and provide direct nursing care and assessments.

Responsibilities include coordinating health services, developing care plans, training on medications and care tasks, and contributing to the Interdisciplinary Team to support

Qualifications

• Active, unrestricted New York State license as a Registered Professional Nurse.
• Bachelor’s degree and a minimum of 3 years clinical experience in a hospital or primary care setting preferred.
• Experience with the Intellectually Disabled population preferred.
• Managerial experience preferred.
• Excellent verbal and written communication skills.
• Ability to work in a team environment.

Responsibilities

• Provide direct nursing care as prescribed by physician/Nurse Practitioner.
• Perform regular health assessments and updates to care plans.
• Supervise LPNs, AMAP and Direct Support Professionals in nursing tasks.
• Coordinate all acute and ongoing health services.
• Conduct training on medication administration and health measures.
