Job summary

Jewish Family Service Association of Cleveland is seeking a full-time Registered Nurse Case Manager to provide in-home nursing care. You will assess patients, document progress, educate families, and coordinate with a multidisciplinary team to support quality outcomes.

The role requires an Ohio RN license, a 4-year degree, and at least two years of clinical experience, including home or hospice care. Travel across the service area and an on-call rotation are involved.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license (OH).
• Two years of prior clinical experience.
• One year of in-home or hospice setting.
• Reliable daily transportation.
• BLS/CPR certification.

Responsibilities

• Completes and submits required documentation, including OASIS, within defined time frames.
• Documents each clinical visit with assessments, vitals, medication review, and education.
• Performs nursing procedures and treatments within scope of a RN.
• Observes and reports changes to supervisor and physicians.
• Coordinates with other health care providers involved in the patient’s care.
• Maintains professional communication with clients and families.
• Teaches clients and families about medications and disease management.
• Travels to provide nursing services across the geographic market.
• Provides on-call support to ensure 24-hour staff availability.
