Job summary

Qualifications

• Associate’s Degree in Nursing for experienced candidates.
• BSN for new graduates.
• NYS RN license required.
• CCM preferred.
• 1 year of RN experience in managed care, home care, acute care or nursing home preferred.

Responsibilities

• Provide case management to members addressing medical, social, psychological, physical and spiritual needs.
• Develop, implement and monitor care plans in collaboration with the PCP, caregivers and other team members to meet member needs.
