University Health KC is seeking a dedicated Case Manager (RN) at Truman Medical Center to coordinate care across the continuum, assess patient needs, and develop personalized care plans. This role focuses on optimizing outcomes, resource use, and patient advocacy.

You will collaborate with patients, families, and the care team, monitor progress, document thoroughly, and facilitate transitions of care to reduce readmissions.

Qualificações

• BSN or an equivalent combination of education and experience.
• Current MO RN license.
• 2+ years of acute care experience.
• Exceptional verbal, telephonic, electronic, and written communication skills.
• Proficiency with Word, Outlook, and Excel.

Responsabilidades

• Assess patient needs and determine eligibility for case management services.
• Create individualized care plans with patients and families.
• Monitor progress and adjust care plans for outcomes.
• Advocate for services and resources for patients.
