Job summary

HCA Healthcare's Medical City Dallas is seeking an RN Case Manager to coordinate patient care across the stay, promoting a safe discharge and efficient transition to post-acute resources.

You will collaborate with physicians, Social Services, and the broader care team to align patient needs with available resources, optimize outcomes, and ensure quality, cost-effective care for a diverse patient population.

Qualifications

• RN license with TX requirements
• Bachelor’s degree preferred; ADN/diploma required as listed
• InterQual experience preferred

Responsibilities

• Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family
• Reassesses the patient’s clinical condition and mitigates readmission risk
• Coordinates the plan of care and drives the discharge plan with the multidisciplinary team
• In partnership with Social Services, ensures post-acute medical needs and appropriate level of care
• Makes timely referrals to Social Services for psychosocial determinants of health
• Involves patient/family in identifying needs and goals
• Evaluates progression using InterQual criteria and escalates as needed
• Makes referrals to third party payer and disease/case management programs
• Facilitates patient throughput and effective care transitions
• Documents recommendations and discharge plans to all care team members
• Aligns patient needs with resources for safe discharge
• Acts as liaison between physicians, patient, family, hospital staff, and outside agencies
• Addresses needs of under-resourced patient populations with education and community resources
• Demonstrates knowledge of regulatory requirements and ethics/compliance
• Advocates for patients’ rights and cultural needs
