Job summary

Sibley Memorial Hospital in Washington, D.C., a member of Johns Hopkins Medicine, is a 318-bed not-for-profit community hospital offering medical, surgical, obstetric, oncology and orthopaedic care, plus a 24-hour emergency department.

The Case Manager collaborates with patients, families, social workers and the interdisciplinary team to coordinate care across the continuum, managing a caseload from pre-admission through post-discharge, removing barriers and developing sustainable transition

Qualifications

• Completion of an accredited BSN Nursing Program (RN) required.
• Minimum 2 years of experience in clinical nursing required; minimum of one-year in case management, and/or discharge planning preferred.
• Accredited Case Manager i.e. – Registered Nurse (ACM – RN) / Certified Case Manager (CCM) preferred
• Licensed to practice as a Registered Nurse in the District of Columbia.
• CPR- Basic CPR required

Responsibilities

• Collaborate with patients, families, social workers, physicians and the care team to coordinate patient-centered care across the continuum.
• Manage clinical resources and transition planning from pre-admission through post-discharge for a caseload.
• Identify and eliminate barriers to the delivery of clinical services and develop safe, sustainable transition plans with patients and families.
