Cynet Health is seeking an RN Case Manager for a 13-week assignment in San Antonio, TX. The role focuses on coordinating care, discharge planning, and seamless transitions across the continuum of care, collaborating with an interdisciplinary team to optimize outcomes and resource use.

The ideal candidate will have a minimum of 2 years in RN Case Management, experience in long-term or short-term acute care settings, and a valid RN license with BLS certification.

Formación

• Minimum 2 years of RN Case Management experience.
• Experience in a long-term acute care or short-term acute care hospital setting.
• Experience with care transitions and discharge planning.

Responsabilidades

• Coordinate and facilitate patient care through effective collaboration with the interdisciplinary team.
• Follow patients throughout the continuum of care and ensure optimal resource use and service delivery.
• Ensure compliance with external review agencies and provide ongoing assessment, care planning, and evaluation of patient needs.
