HCA Healthcare's Methodist Hospital in San Antonio, TX is seeking a dedicated Case Manager to support patients and families through every step of their care journey. You will coordinate services, develop individualized care plans, guide discharge planning, and document psychosocial assessments reflecting each patient's unique needs.

• Graduate of an accredited nursing program (Diploma/AS/BS/BSN).
• Minimum 3 years clinical experience in case management or related area.
• Preferred: utilization review, resource management, discharge planning experience.

Responsabilidades

• Guide patients and families through orientation, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers for participation in treatment planning.
• Develop and implement individualized treatment plans reflecting patient strengths and needs.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings clearly.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values of compassion, respect, and excellence in every interaction.
