Methodist Hospital in San Antonio is seeking an experienced Case Manager to support patients and families through the care journey. You will coordinate services, develop care plans, and collaborate with physicians, nurses, and leaders to ensure safe transitions.

This role requires an RN license, a nursing degree, and at least 3 years of clinical experience, with preference for utilization review or discharge planning experience. Strong communication and teamwork are essential.

Qualifications

• Graduate of an accredited diploma, associates, or baccalaureate degree nursing program.
• Minimum of 3 years clinical experience in area in which Case Management will be performed.
• Prefer at least one year experience in utilization review, resource management, discharge planning or case management.

Responsabilités

• Guide patients and families through orientation, explain benefits, discharge criteria, and patient rights.
• Coordinate education for patients and families to encourage participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and goals.
• Complete psychosocial assessments and discharge planning evaluations and document findings clearly.
• Collaborate with the Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote values of compassion, respect, and excellence in every interaction.
