Job summary

Elliot Hospital in Manchester, NH is seeking an experienced RN Case Manager to provide clinically-based case management and coordinate safe transitions of care. Under the supervision of the Director of Case Management, you will assess discharge needs and collaborate with the health care team to identify resources and ensure a smooth transition for patients.

The role requires a Bachelor's degree in a healthcare-related field (BSN preferred) and 3–5 years of case management experience; CCM or ACM

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• 3–5 years of case management-related healthcare experience.
• CCM or ACM credentials preferred.

Responsibilities

• Assesses and identifies patients’ needs during the discharge planning process.
• Provide resources and community referrals for continuity of care.
• Care coordination with families, providers, and clinical team.
• Follows Medicare conditions when needed.
• Utilization reviews.
