Strong communication and organizational skills are essential. Responsibilities include collaborating with the healthcare team, connecting patients to community resources, and conducting chart reviews to ensure appropriate utilization.

Qualifications

• Bachelor's degree in nursing required; Master's preferred.
• Licensed as a Registered Nurse in the State of New Hampshire.
• BCLS certification required within 30 days of hire.
• Board Certification in Case Management preferred; willingness to pursue CCM.
• Five years experience in a health care setting; case management a plus.
• Excellent interpersonal and communication skills; ability to multi-task.

Responsibilities

• Lead a collaborative process with the healthcare team to coordinate patient care and transitions.
• Promote patient and family-centered care coordination and transitions.
• Facilitate connections to community resources for patients and families.
• Organize patient care activities to optimize resource use.
• Coordinate integration of social services into discharge planning.
• Conduct chart reviews and staff education on utilization and discharge planning.
• Promote effective utilization of health care services and assist with payers questions.
• Assess patient progress and advocate for patient and family decisions.
• Facilitate rounds and conferences to review goals and post-hospital needs.
• Develop care plans with clinical staff and ensure continuity of social services.
