Job summary

HonorHealth is seeking an RN Case Manager at Osborn Medical Center in Scottsdale, AZ. The role coordinates care for high-risk patients and guides the discharge process for complex post-acute needs.

You will analyze medical records for admission and length of stay, advocate for patients, and collaborate with physicians and the care team to ensure timely, safe care and appropriate resource use.

Qualifications

• 3 years clinical experience in a hospital setting (required)
• 5 years in RN Case Management (preferred)
• 7 years as RN (preferred)
• RN license in AZ or state compact (required)
• CCM certification preferred for Case Management

Responsibilities

• Guides care coordination for high risk acute care patients and facilitates discharge planning.
• Analyses medical records for admission legitimacy, treatment, and LOS.
• Advocates for patients/families and liaises with the care team.
• Collaborates with physicians to coordinate resources for timely services.
• Coordinates health services and referrals to ensure safe discharge.
