Qualifications

• Education: Bachelor's or Associate degree in nursing; RN licensure in Arizona.
• Experience: Two years nursing or case management; NICU requires three years NICU nursing experience.
• Licensure/certification: BLS mandatory; some departments may require CPR instructor, NRP.

Responsibilities

• Collaborate with multidisciplinary teams in discharge planning across all phases of care.
• Assess, create, implement, evaluate and modify discharge plans.
• Coordinate case management with payors and healthcare providers.
• Ensure timely, accurate documentation in the EMR.
• Assist patients and families in decision making while honoring preferences.
