Job summary

Naples Comprehensive Health - NCH is seeking an RN Case Manager to advocate for patients and coordinate their care across settings. The role focuses on developing care plans, ensuring efficient throughput, and engaging families in goal-oriented care.

You will participate in multidisciplinary rounds, maintain regulatory compliance, and collaborate with the care team to support safe discharges and optimal utilization of resources.

Qualifications

• Minimum of Associate Degree in Nursing required; BSN preferred.
• Minimum of 1 years Discharge planning, case management, managed care, or Registered Nurse experience in a medical setting.
• Licensed as a Registered Nurse (RN) in the state of Florida.
• Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
• Case Management Certification preferred.

Responsibilities

• Screen all patients for clinical, psychosocial, financial, and other factors affecting care and collaborate with patients/families in goal setting.
• Evaluate understanding and engagement with progress toward goals and adjust the plan of care accordingly.
• Arrange services among community agencies, providers, patients and others involved in the plan of care.
• Provide patients/families with tools and resources to make informed choices.
• Develop clinically appropriate care plans aligned with patient goals and available resources.
• Facilitate communication and ensure transition plans are implemented and shared with stakeholders.

Education

Associate Degree in Nursing

BSN preferred

RN licensure in Florida

Case Management Certification preferred
