Job summary

Cape Fear Valley Medical Center in Fayetteville, NC is seeking a Nurse Case Manager to plan discharge, coordinate services, and ensure care is delivered across the continuum. You will work with physicians and an interdisciplinary team to facilitate guidelines and desired treatment outcomes.

Responsibilities include early discharge planning, arranging post-acute care, interviewing patients/families, and contributing to quality improvement activities to enhance patient outcomes.

Qualifications

• Associate degree required for RN licensure in NC.
• Bachelor's degree in Nursing preferred for advancement.
• Current RN licensure in NC or Compact state.
• Professional Case Management Certification (CCM or ACM) preferred.
• 3 years’ Acute Care experience required.
• 1 year Med/Surg/ICU experience preferred.
• 1 year Case Management experience preferred.
• 1 year Home Health/Hospice experience preferred.

Responsibilities

• Review patient records to verify observation or admission status.
• Evaluate chart for clinical appropriateness of continued stay.
• Assign anticipated discharge date within 24 hours of admission.
• Conduct initial interview with patient/family to plan care and coordination of services.
• Assist patient/family in coping with hospitalization and chronic illness.
• Educate patient/family on community resources and post-acute care options.
• Coordinate discharge planning and arrangements for post-acute services.
• Identify refer cases to Leadership Team for escalation as needed.
• Proactively prevent delays in care delivery.
• Serve as a resource for guardianship, abuse, and advance directives issues.
• Consult with Social Work Case Managers on Medicaid and disability referrals.
• Precept new RN Case Managers as designated by Leadership Team.
• Participate in quality improvement activities to promote best practices.
