Qualifications

• Bachelor's degree in Nursing and two years’ case management work experience required.
• Master’s Degree Preferred.
• Licensure as a registered nurse by the state of South Carolina or compact state required.
• Case Management Society of America (CSMA) or American Case Management Association (ACMA) certification preferred.
• Must obtain certification within 1 year, if not currently certified.
• Master’s Degree in Social Work and licensure by South Carolina LLC; Supervisory experience preferred.

Responsibilities

• Provide professional supervision and direction to care team members (RNs, MSWs, Program Coordinators).
• Serve as clinical expert and resource for Case Management, discharge planning and care transitions.
• Aim for best clinical outcomes for patient groups.
