The candidate should hold a Bachelor's degree in Nursing or Social Work, with RN preferred and LVN/LPN acceptable. Knowledge of Medicare/Medicaid is essential, and CCM certification is a plus for advancing in this role.

Qualifications

• Bachelor's degree in nursing or social work is required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid.
• CCM certification a plus.
• Must maintain continuing education/licensing.

Responsibilities

• Communicate regularly with residents and families about Plans of Care, PT, OT and other treatment protocols.
• Attend morning meetings to facilitate communications with the team.
• Answer residents' questions about care, treatment plans and illness progression.
• Monitor and adjust residents' statuses based on changing needs.
• Organize and prioritize daily work for residents' needs.
• Complete documentation as required.
• Perform utilization review activities for cost-effective care.
• Coordinate care with resident, providers, facilities, payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Relate positively with residents, families, staff and professionals.
• Accurate charting and timely paperwork.
• Work independently with sound judgement in interactions with physicians and payers.
