Job summary

Southern Indian Health Council is seeking a Registered Nurse Case Manager (RNCM) to deliver care management for patients with chronic illness and mental health needs. You will manage care plans, coordinate with providers, and liaise with facilities to improve access to services.

Responsibilities include triaging referrals within 24 hours, documenting in the NextGen EHR, and collaborating with a broad healthcare team to optimize outcomes.

Qualifications

• Must possess a current California Registered Nurse license.
• Minimum of 3 years’ experience in a similar healthcare role.
• HIPAA knowledge in ambulatory care setting, with confidentiality maintained.
• CPR current; ACLS must be obtained in the first four weeks of employment.

Responsibilities

• Assess patient physical and mental health needs to develop individualized care plans.
• Accept referrals from multiple sources and triage according to medical necessity within 24 hours.
• Coordinate with multidisciplinary team to implement and adjust care plans as patient progress dictates.
• Document progress and referrals in the NextGen EHR.
• Ensure patients are active participants in their treatment and understand their plan of care.
• Provide and coordinate patient care across clinics during staffing shortages as needed.
