Incredible Health is actively hiring for a Nurse Case Manager (RN) in the Grand Blanc, MI area. The role focuses on clinical pathway coordination, navigator support, and utilization review, with day shifts available and options for full-time, part-time, or per diem employment.

The ideal candidate will hold a BSN or higher, be actively licensed RN, and bring at least 1 year of case management experience within a healthcare setting.

Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered Registered Nurse license.
• 1+ years experience in case management in healthcare or hospital settings.
• Exceptional interpersonal and communication skills to collaborate with clinicians, patients and families.
• Proficiency in electronic health records (EHR) software.

Responsabilités

• Conduct comprehensive assessments of patients, including medical history, medications, and psychosocial needs.
• Collaborate with healthcare providers, patients and families to develop and implement individualized care plans.
• Monitor patient progress and adjust care plans as needed, communicating changes to the care team.
• Serve as the main point of contact between patients and healthcare providers for seamless coordination.
• Maintain up-to-date records and ensure confidentiality and compliance.
