Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered Registered Nurse license.
• 1+ years experience in case management, preferably in healthcare or hospital setting.
• Exceptional interpersonal and communication skills, both written and verbal, to collaborate with professionals, patients, and family members.
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive assessments of patients, including medical history and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement care plans.
• Monitor and evaluate patient progress, adjusting plans as needed and communicating changes to the team.
• Serve as main point of contact between patient and healthcare providers for coordination of care.
• Maintain up-to-date records and case notes, ensuring confidentiality and compliance.
