Qualifications

• Graduate of an accredited Nursing Program.
• Three (3) years of clinical experience in healthcare or recent case management experience.
• Strong leadership ability, good organization skills, independent and critical thinking skills, sound judgment.
• Strong ability to communicate complex topics to a wide audience.
• Proficient documentation skills.
• Knowledge of Payor/Insurance benefits.
• Functional skills on PC and related software (Microsoft Office).
• Case Management Certification preferred.

Responsibilities

• Discharges planning and care coordination across the care continuum.
• Facilitates appeals/grievances and collaborates with payors to resolve issues.
• Performs initial utilization review for medical necessity and documents findings.
• Communicates with Attending Physician and healthcare team to advance the plan of care.
• Maintains patient rights and HIPAA compliance during discharge processes.
• Supports continuity of care through accurate data entry and reporting.
• Travels between Independence Health locations as needed.
