Qualifications

• Graduate of an accredited school of nursing.
• Active RN license to practice.
• 2 years of acute hospital case management experience.

Responsibilities

• Facilitates care coordination along a continuum to promote timely throughput and appropriate utilization of resources.
• Precepts new staff and participates in department quality improvement initiatives; TEMPO program involvement as needed.
