Responsibilities

• Screen medical necessity and submit for Physician Advisor review.
• Coordinate care and transition planning.
• Lead multidisciplinary patient care conferences.
• Assist in complex case reviews and referrals to Social Work.
• Ensure timely and accurate documentation in the Case Management system.
• Communicate plan of care with patients and families.
• Collaborate with physicians, staff, and ancillary departments.
