Job summary

HCA Florida Bayonet Point Hospital in Hudson, FL is seeking a Case Manager RN to support patients and families through their care journey, coordinating services, resources and care plans. You will partner with physicians, nurses and the care team to ensure safe transitions and clear communication while promoting HCA Healthcare values of compassion, respect and excellence.

Requirements include ADN (BSN preferred), RN license, 2+ years in case management or 3+ years nursing, InterQual experience

Qualifications

• ADN required; BSN preferred.
• 2+ years in case management or 3+ years in clinical nursing.
• Current RN license in applicable state(s).
• InterQual experience preferred; CM certification preferred.
• Ability to collaborate effectively and communicate clearly.

Responsibilities

• Guide patients and families through program orientation, discharge planning, and education.
• Coordinate education for patients, families, and caregivers for treatment planning.
• Develop and implement individualized treatment plans reflecting patient goals.
• Complete psychosocial assessments and document findings for the care team.
• Collaborate with Rehab Program Director and care team to ensure seamless care.
