Ascension is seeking a Case Management RN in Nashville, TN to lead care planning across the continuum. You will assess patient needs, monitor medical necessity and levels of care, advocate for resources, and collaborate with the care transition team to ensure smooth post-discharge arrangements.

This on-site, full-time role in the Case Management department emphasizes patient-centered transitions and documentation accuracy.

Qualifikationen

• Licensed Compact State RN credentialed from Tennessee Board of Nursing before hire.
• BLS Provider certification preferred by American Heart Association or Red Cross.

Aufgaben

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Monitors patient medical necessity and level of care through assessments and records.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team for effective implementation and reevaluation of health plan.
• Acts as a resource for medical record documentation and services related to diagnoses and post-discharge care.
