Job summary

Heart to Heart Hospice is seeking an RN Case Manager in Texas to assess and coordinate care for patients facing terminal illness. You will partner with families and the care team to deliver compassionate, compliant, person-centered plans.

The role requires an RN license, a nursing degree, a valid driver’s license, and reliable transportation. On-site position with competitive benefits and opportunities for growth within the hospice program.

Qualifications

• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Conduct initial and ongoing assessments of the impact of the terminal disease on the patient’s physical, psychosocial and environmental needs and ADLs.
• Implement the individualized plan of care and revise the plan as necessary.
• Collaborate with patient, caregiver, physicians and other healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and coordinate patient care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients and families.
• Submit documentation in a timely manner.
• Perform other duties as required and ensure compliance with laws and policies.
