Qualifications

Responsibilities

• Assess patients' medical, psychosocial, and environmental needs to develop personalized care plans.
• Coordinate with physicians, nurses, social workers and other professionals to implement and monitor care plans.
• Facilitate communication between patients, families and providers to ensure understanding and adherence to treatment protocols.
• Identify and connect patients with community resources and follow-up care to promote recovery and wellness.
• Document all case management activities accurately and maintain regulatory and organizational standards.
