Job summary

Qualifications

• Must have strong foundation in case management and medical case management.
• Experience with discharge planning and care transitions across inpatient, outpatient, and post-acute settings.
• Knowledge of utilization management including review processes and length of stay management.
• Understanding of Medicare regulations and reimbursement related to hospital case management.
• Licensed clinical background (RN, LCSW or related) with acute care experience preferred.
• Proven leadership and staff supervision with coaching and performance improvement.
• Excellent communication and collaboration with interdisciplinary teams and external partners.
• Strong analytical and organizational skills with data-driven process improvements.
• Familiarity with EHRs and case management or utilization review software.
• Bachelor’s degree in nursing, social work or related field; advanced degree or CCM/ACM certification preferred.

Responsibilities

• Oversee hospital case management functions and ensure safe patient care transitions.
• Lead and supervise the case management team and coordinate discharge planning.
• Collaborate with physicians, nursing, social services and other departments to promote continuity of care.
• Monitor utilization management activities, length of stay and payer requirements.
• Develop and update case management policies and educate staff on best practices.
• Participate in quality improvement initiatives and strategic planning for patient flow and resource use.
