LMH Health is seeking an experienced RN Case Manager to coordinate patient care from assessment through discharge. You will collaborate with patients, families, physicians, and the healthcare team to promote quality, cost-effective outcomes.

The role emphasizes data gathering, goal-setting, plan modification, and advocacy for patient needs, with a strong focus on resource management and interdepartmental communication.

Qualifications

• Registered Nurse with current Kansas nursing license.
• At least one year of hospital case management and/or discharge planning experience.
• Experience or familiarity with InterQual is preferred.

Responsabilités

• Gather relevant data from chart review and patient interviews.
• Identify barriers, clarify goals, and seek potential alternatives.
• Develop treatment and discharge plans with patient, family, physician, and healthcare team to promote positive outcomes and cost-effective care.
• Initiate and modify plans based on treatment response through continuous monitoring and communication with the healthcare team.
• Promote communication between healthcare team, patient/family, and physicians.
• Monitor resource use, patient status, and level of care.
• Communicate with third-party payors to ensure payment and maximize benefits.
• Educate patient/family, physician, and healthcare team on available resources and regulations.
• Represent patient and hospital interests in funding, treatment options, and timelines.
