Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ years of case management experience, preferably in healthcare or hospital setting.
• Excellent interpersonal and written/verbal communication to collaborate with medical professionals, patients, and families.
• Proficiency with electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive patient assessments including medical history, medications, treatment plans, and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans.
• Monitor and evaluate patient progress, adjust care plans as needed, and inform the healthcare team of changes.
• Serve as the main point of contact between patient and healthcare providers to ensure seamless communication and care coordination.
• Maintain up-to-date records and case notes in a confidential, compliant manner.
