Incredible Health in Grand Blanc, MI is actively hiring for a Nurse Case Manager (RN) to support clinical pathways, navigator tasks, and utilization review at our network hospitals.

We offer day shifts with full-time, part-time, and per diem options. The ideal candidate holds a BSN, active RN license, and 1+ year case management experience, with strong communication and EHR proficiency.

Qualifications

• BSN or higher from an accredited program.
• Active RN license in good standing.
• 1+ year case management experience in healthcare.
• Strong written and verbal communication; proficient with EHR.

Responsibilities

• Conduct comprehensive patient assessments including medical history and psychosocial needs.
• Collaborate with providers, patients, and families to develop individualized care plans.
• Monitor patient progress and adjust plans as needed.
• Serve as main contact between patient and providers to coordinate care.
• Maintain confidential records and compliant notes.
