Huntsville Hospital is seeking an RN Case Manager to support the physician and interdisciplinary team in coordinating patient care and improving outcomes while managing costs. The role integrates utilization management, care coordination, and discharge planning to ensure timely, accurate information for payers and efficient resource use.

The ideal candidate is a graduate of an accredited nursing program with active Alabama RN license (or multi-state) and at least three years of acute care

Formación

• Graduate of an accredited school of nursing.
• Current AL RN or Multi-State license.
• Three years acute care experience preferred.

Responsabilidades

• Support the physician and interdisciplinary team to facilitate patient care.
• Coordinate Utilization management, care coordination and discharge planning.
• Manage a designated patient caseload and plan to meet patient needs and optimize length of stay.
