Job summary

Baptist Health, a leading healthcare system in central Alabama, seeks an RN Case Manager to coordinate patient care across the continuum. The role involves discharge planning, utilization management, and collaboration with physicians and teams to ensure efficient resource use and high-quality outcomes.

Required is an RN license with a Bachelor's degree in Nursing; case management experience and certification are preferred.

Qualifications

• Active RN license required. Case management certification preferred.
• Experience in hospital case management, utilization review, or home care.
• Strong computer, oral and written communication skills; ability to work independently.

Responsibilities

• Coordinate care with physicians, nursing staff and other professionals to optimize clinical resources and outcomes.
• Complete admission, continued stay and discharge reviews per regulations and guidelines.
• Support revenue protection by reconciling orders, bed billing type and medical necessity.
