Job summary

Heart to Heart Hospice is seeking an RN Case Manager in Texas to perform initial and ongoing assessments of patients with terminal illness, coordinate the care plan, and collaborate with the care team to meet medical and psychosocial needs.

Role includes on-call coverage, documentation, and ensuring compliance with regulations and policies, with a focus on patient-centered care and family support.

Qualifications

• Two years of RN experience preferred in a healthcare/clinical setting.
• Current State RN license.
• Current valid driver's license and reliable transportation, with ongoing auto liability insurance.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Collaborates care amongst patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with prioritization of needs.
• Perform on-call responsibilities and provide on-call service to patients and families as assigned.
• Timely submission of all documentation.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established policies and procedures.
