Qualifications

• Graduated from an approved and certified RN program.

• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines for long-term care.

Responsibilities

• Managing care for residents through pre-admission, admission, and post-admission stages.
• Coordinating services to meet residents' needs in a timely and effective manner.
• Collaborating with healthcare providers, families, and interdisciplinary teams to adjust care plans.
• Monitoring residents' progress and ensuring appropriate interventions are implemented.
• Facilitating smooth transitions of care with necessary services and resources.
• Advocating for residents and families and providing education as needed.
